This case discusses a giant partially thrombosed and partially calcified aneurysm arising from the left middle cerebral artery (MCA) in a 48-year-old female patient. Summary: The patient presented with intermittent headache and right hemiparesis for six years. Imaging studies, including non-contrast computed tomography, magnetic resonance imaging, and digital subtraction angiography, confirmed the presence of a significant aneurysm with both thrombosed and calcified components. Surgical intervention involved trapping the aneurysm and performing a superficial temporal artery to MCA bypass. Conclusion: The case highlights the complexity of managing a giant partially thrombosed and calcified aneurysm, particularly regarding surgical approaches. The patient had a successful postoperative outcome, maintaining her preoperative clinical condition.
