Isaacs","William Morrish","Naomi C. Gallagher"],"abstract":"Background: Epidural hematomas (EDH) can occur in various locations, with vertex epidural hematomas (vEDH) being relatively uncommon. These injuries may lead to significant morbidity and mortality if not promptly identified, particularly due to potential involvement of the superior sagittal sinus. Summary: We present a case of a 26-year-old male who sustained blunt force trauma to the head, resulting in a large vEDH. Imaging revealed mass effect on the brain and downward herniation, with indications of active extravasation. The patient underwent emergency surgery, which successfully evacuated the hematoma and restored neurologic function. Conclusion: This case highlights the importance of timely diagnosis and management of vEDH, particularly in the context of potential superior sagittal sinus injury. Postoperative imaging confirmed successful evacuation and resolution of herniation, with the patient recovering full neurologic function.
