Goldsmith"],"abstract":"Background: Binswanger disease is a progressive form of dementia characterized by excessive arteriosclerosis in cerebral arteries, leading to restricted cerebral blood flow (CBF). Unlike Alzheimer's disease (AD), where neuron death is believed to cause decreased CBF, Binswanger disease is primarily due to narrowed arterial lumens. Improved MRI techniques have enhanced the diagnosis of Binswanger disease, which may have previously been misclassified as AD. Methods: A study was conducted on twenty cynomolgus monkeys fed an atherogenic diet for 12 months, with five monkeys subjected to induced hypertension. Following the diet, various arterial segments were excised and examined histologically. The assessment focused on the presence of atherosclerosis in different arteries, including omental arteries. Results: All monkeys developed severe atherosclerosis in major arteries, particularly those subjected to hypertension. In contrast, omental arteries remained normal in appearance and were devoid of atherosclerosis, confirmed through radiological and histological examinations. Conclusion: Given the absence of atherosclerosis in omental arteries, placing a vascularized omental pedicle on the brain of patients with Binswanger disease may enhance CBF to subcortical areas. This approach warrants further investigation through controlled studies to evaluate its potential therapeutic benefits.
