Case Description: A 72-year-old male with a history of cervical spondylotic myelopathy (CSM) presented quadriparetic after a fall. The cervical magnetic resonance imaging revealed CSM plus posterior AAD associated with marked backward displacement of an odontoid type II fracture. After reduction of the odontoid process, C1 to T2 screw-hook-rod fixation and a subaxial cervical laminectomy/multilevel fusion resulted in marked neurological recovery. Conclusion: Here, a 72-year-old male with underlying CSM presented with the acute onset of quadriparesis following a fall. Having sustained backward displacement/ptosis secondary to posterior AAD from the hyperextension injury, the patient successfully underwent a combined closed/open reduction of the displacement combined with subaxial decompression and long segment fixation.","thumbnailUrl":"https://sni-digital-videos.s3.amazonaws.com/articles/c2d082b3-765f-4df6-985c-13d133722dd1/featured/hero-1781558057727.png","publishDate":"2025-12-26T00:00:00.000Z","doi":"10.25259/SNI_844_2025","categories":["Spine","Case Report"],"fullTextUrl":"https://surgicalneurologyint.com/wp-content/uploads/2025/12/14201/SNI-16-546.pdf"}
