The 'crown and necklace' appearance is a notable radiological finding associated with these lesions. Summary: We present a case of a 35-year-old female who exhibited headaches and fever, with neurological examination revealing no deficits. MRI scans demonstrated multiple ring-enhancing lesions in the suprasellar and interpeduncular cisterns, consistent with the 'crown and necklace' appearance, alongside laboratory findings indicative of tuberculosis. Conclusion: The identification of the 'crown and necklace' appearance on MRI is crucial for the diagnosis of cisternal tuberculomas. Early recognition and treatment are essential, although follow-up in this case was not possible.
