This article discusses the implications of delayed imaging in the context of a patient who experienced intraoperative acute brain swelling. Methods: The authors of the referenced article described a case involving a 28-year-old male with a traumatic left frontal contusion and right temporal bone fracture. The patient underwent left-sided hemicraniectomy and evacuation of an intracerebral hematoma, with subsequent imaging delayed due to hemodynamic instability. Results: The delay in obtaining a computed tomography (CT) scan postoperatively has been highlighted as a critical factor, as it may have impacted the detection of contralateral hematoma. The clinical findings, including anisocoria and clouding of consciousness, were noted as late manifestations of transtentorial herniation. Conclusion: This case underscores the importance of immediate postoperative imaging in patients with intraoperative complications. The authors advocate for the development of protocols to enhance patient care in trauma settings, particularly when adverse events occur.
