These include cranial nerve dysfunction, motor deficits, and hydrocephalus, which can significantly impact patient quality of life. The optimal treatment for vertebrobasilar dolichoectasia remains uncertain due to a lack of systematic reviews and long-term data on surgical interventions. Methods: A thorough neurological examination and neuroradiological investigations are essential for diagnosing vertebrobasilar dolichoectasia and planning surgical interventions. Magnetic resonance imaging with specific three-dimensional sequences enhances visualization of cranial nerves and cerebral vessels, aiding in the diagnosis of neurovascular compression disorders. Results: Surgical intervention, particularly microvascular decompression (MVD), has been identified as an effective treatment for symptomatic neural compression caused by dolichoectatic vertebrobasilar arteries. Careful surgical techniques are necessary to minimize the risk of injuring cranial nerves and perforating vessels during vessel repositioning. Conclusion: The management of dolichoectasia of the vertebrobasilar complex requires careful assessment of symptoms and surgical planning to ensure effective relief of neural compression while preserving neural and vascular integrity. Continued research and case studies are needed to establish standardized treatment protocols.
