The authors highlight that certain surgical techniques, such as craniectomy and reopening procedures, are significantly associated with an increased risk of CSF leak. Summary: In this letter, we emphasize the importance of the fascia opening method and closure technique in preventing CSF leaks. We advocate for a Y-shaped opening of the fascia, which is not reflected from the bone, and a watertight closure that enhances the seal. Additionally, we acknowledge that postoperative hydrocephalus and nonwatertight dural closure contribute to CSF leaks. Conclusion: Our observations suggest that meticulous attention to the fascia opening and closure techniques may reduce the incidence of CSF leaks following retrosigmoid surgery. Further studies may be warranted to validate these findings.
