Advances in imaging technologies, such as functional magnetic resonance imaging (fMRI) and positron emission tomography (PET), allow for enhanced analysis of brain function and its correlation with anatomy. This has opened new avenues for neuromodulation therapies that were previously unexplored in neurosurgery. Methods: The article discusses the integration of various imaging modalities and electrophysiological techniques to study peripheral nerve stimulation's effects on brain function. It emphasizes the importance of methodical studies and clinical trials to understand the therapeutic possibilities of peripheral nerve stimulation. The trigeminal nerve is highlighted as a key conduit for modulating brain function due to its accessibility and established role in treating pain. Results: Stimulation of the trigeminal nerve has been utilized for various conditions, including refractory facial pain, depression, and epilepsy, although the mechanisms of action remain poorly understood. The article notes that functional imaging can now detect the influence of peripheral nerve stimulation on brain activity, leading to potential new therapeutic approaches. The relationship between peripheral nerve stimulation and functional imaging is emphasized as a critical area for future research. Conclusion: The article underscores the potential of peripheral nerve stimulation, particularly through the trigeminal pathway, to offer new therapeutic options for disorders affecting cognition, emotion, and sensory systems. By leveraging advances in imaging and clinical research, stereotactic neurosurgeons can develop innovative surgical procedures and therapies. The future of neurosurgery is promising, with the integration of these technologies paving the way for enhanced patient care.
