Recent studies suggest that dexmedetomidine may offer protective effects on mitochondrial enzymes and reduce inflammation and neurotoxicity. Summary: This letter presents an integrated dexmedetomidine-sevoflurane algorithm for anesthetic induction in neurosurgery, particularly in patients with mitochondrial dysfunction. The algorithm was successfully implemented in three patients undergoing spine surgeries, achieving loss of consciousness within 10 minutes and stable hemodynamics during the procedure. Conclusion: The integrated dexmedetomidine-sevoflurane algorithm demonstrates potential as a viable anesthetic strategy in neurosurgery, particularly for patients with mitochondrial dysfunction, providing adequate parameters for safe and effective induction.
