The blood-brain barrier (BBB) and blood-brain tumor barrier (BBTB) severely limit the effectiveness of chemotherapy by preventing drug penetration into brain tissue. This editorial proposes a novel surgical approach utilizing the omentum to bypass these barriers and enhance drug delivery directly to the tumor site. Summary: The proposed operation involves placing a vascularized omentum into a cavity created by the excision of a glioblastoma. This technique aims to develop omental arteries that can penetrate brain tissue and facilitate the delivery of chemotherapeutic agents, potentially improving treatment outcomes. The omentum's angiogenic properties, due to high levels of vascular endothelial growth factor (VEGF), are critical for this approach. Conclusion: The introduction of the omentum into the brain cavity presents a promising strategy to enhance chemotherapy delivery for glioblastoma patients. The success of this operation may depend on the strength of the chemotherapy administered through the omental arteries, which are devoid of BBB and BBTB. Given the aggressive nature of glioblastomas, urgent clinical investigation is warranted.
