Previous studies indicated a lower prevalence of blood group A in patients with Hodgkin's and non-Hodgkin's lymphoma, as well as in primary central nervous system lymphoma (PCNSL). This study aims to evaluate the frequency of blood group A in patients with secondary central nervous system lymphoma (SCNSL). Methods: The study included 202 patients diagnosed with SCNSL across three referral hospitals in Tehran. Diagnosis of CNS involvement was confirmed through patient history, clinical examination, imaging studies, and cerebrospinal fluid analysis. Blood group distribution among patients was compared to that of 1000 healthy Iranian controls. Results: The blood group distribution among patients with SCNSL was significantly different from that of healthy controls, with blood group A present in only 5.0% of patients. The O:A:B:AB ratio among patients was 60 (29.7%):10 (5.0%):125 (61.9%):7 (3.5%), compared to 356 (35.6%):371 (37.1%):212 (21.2%):61 (6.1%) in controls. This study corroborates previous findings of a low frequency of blood group A among lymphoma patients. Conclusion: The findings suggest a low frequency of blood group A in patients with SCNSL, consistent with previous studies on lymphoma. This raises questions about potential protective roles of blood group A against lymphoma, warranting further investigation into the underlying mechanisms.
