Unlike adults, where hypertensive causes predominate, pediatric intracerebral hemorrhage (ICH) is often linked to vascular lesions such as arteriovenous malformations (AVMs) and cavernous malformations (CMs). Proper evaluation of pediatric ICH necessitates advanced imaging techniques to identify potential vascular lesions. Methods: The review discusses the management of pediatric ICH, emphasizing the importance of immediate workup and treatment upon diagnosis. It outlines the use of various imaging modalities, including CT, CTA, DSA, and MRI, to assess the presence of vascular lesions. Surgical interventions are also detailed, focusing on the management of intracranial pressure and the careful approach to hematoma evacuation. Results: Illustrative cases demonstrate the variability in presentation and management of pediatric ICH, highlighting the need for tailored surgical approaches based on individual circumstances. The review notes that AVMs are the most common cause of ICH in children and discusses the implications of their management, including the use of intraoperative imaging techniques. Conclusion: Effective management of pediatric ICH requires a multidisciplinary approach, incorporating advanced imaging and surgical techniques. The review underscores the importance of recognizing and addressing underlying vascular lesions to optimize outcomes in affected pediatric patients.
