Historically, nonabsorbable sutures used for this purpose have led to complications such as nonunion and cosmetic defects. The importance of maintaining the craniofacial region's appearance is critical for patient self-image and overall surgical outcomes. Summary: This letter discusses the use of titanium miniplates for cranial bone flap fixation in a cohort of 71 patients who underwent craniotomies for various indications. The patients were followed for 3–12 months, with assessments showing good cosmetic outcomes and minimal complications. The use of titanium miniplates resulted in reduced operating times and improved fixation compared to traditional methods. Conclusion: The experience reported suggests that titanium miniplates are a cost-effective and effective solution for cranial bone flap fixation in a developing healthcare context. Their use not only enhances functional outcomes but also significantly improves cosmetic results, which is an essential consideration in neurosurgery.
