North","Zain Boghani","Daniel Hansen","Sandi Lam"],"abstract":"Background: Medulloblastoma (MB) is the most common posterior fossa tumor in children, typically presenting with obstructive hydrocephalus and various neurological symptoms. It is classified into four distinct molecular subgroups that correlate with clinical outcomes and histological appearance. Metastatic disease is often present at diagnosis, necessitating comprehensive imaging of the craniospinal axis. Methods: This review article synthesizes current knowledge on the molecular classification of medulloblastoma, surgical management, and treatment outcomes. It discusses the significance of molecular subgrouping in guiding treatment strategies and the implications of surgical resection on patient prognosis. Results: The extent of surgical resection is associated with event-free survival, although the relationship may be influenced by molecular subtype. Hydrocephalus is a common postoperative complication, and risk stratification based on tumor characteristics is essential for determining adjuvant therapy, including radiation and chemotherapy. Conclusion: Modern management of medulloblastoma requires a multidisciplinary approach that incorporates molecular classification to optimize surgical and adjuvant treatment strategies. Continued research into the genetic underpinnings of medulloblastoma is critical for improving patient outcomes.
