Smith"],"abstract":"Background: This article presents key perspectives on various neurosurgical topics, including health-related quality of life in children with spina bifida, the efficacy of cilengitide in glioblastoma treatment, and the management of intracranial metastasis. Each study addresses significant clinical questions and provides insights into patient outcomes and treatment strategies. The overarching goal is to enhance understanding and improve care in neurosurgery. Methods: The studies employed diverse methodologies, including prospective assessments using the Health Utilities Index Mark 3 for spina bifida patients, a multicenter randomized phase 3 trial for cilengitide in glioblastoma, retrospective analyses for surgical intervention outcomes in metastatic brain disease, and Gamma Knife radiosurgery outcomes in Neurofibromatosis Type 2 patients. Surveys were also administered to assess patient misconceptions regarding lumbar spondylosis. Results: Findings indicate that children with myelomeningocele have lower health-related quality of life scores compared to those with closed spinal dysraphism. The addition of cilengitide did not improve overall survival in glioblastoma patients. Surgical resection combined with stereotactic radiosurgery significantly improved time-to-local recurrence. Gamma Knife radiosurgery achieved a 92% local tumor control rate in NF2 patients, although distant treatment failures were noted. Conclusion: The studies highlight the importance of tailored approaches in neurosurgery, emphasizing the need for specific quality of life measures for spina bifida patients and the limited efficacy of cilengitide in glioblastoma. Surgical strategies combining resection and radiosurgery show promise for metastatic brain disease, while Gamma Knife radiosurgery is effective for NF2-associated meningiomas. Addressing patient misconceptions in lumbar spondylosis is crucial for informed treatment decisions.
