It is a rare presenting symptom associated with various neurological conditions, including brain tumors. The relationship between pathological laughter and petroclival meningioma is particularly unusual. Summary: We present a case of a 42-year-old male who experienced pathological laughter for six weeks, followed by additional neurological symptoms. Imaging revealed a left petroclival meningioma causing significant brainstem compression. Following surgical intervention, the patient's pathological laughter resolved completely. Conclusion: This case highlights the importance of recognizing pathological laughter as a potential early symptom of petroclival meningioma. The resolution of symptoms post-surgery underscores the need for timely intervention in similar cases.
