The introduction of vestibulo-ocular monitoring (VOM) using galvanic labyrinth polarization (GaLa) offers a new approach to assess brainstem function and predict outcomes in sTBI patients. Methods: The authors conducted VOM through video-oculographic recording of eye movements during GaLa stimulation in comatose sTBI patients within 3 days of injury. The oculomotor response (OMR) to labyrinth stimulation was recorded to evaluate brainstem function. Results: In a study of 26 patients, GaLa induced OMR in 15 (57.7%) patients, while 11 (42.3%) did not respond. All non-responders had an unfavorable Glasgow outcome score (GOS) of 1–2, with 90.9% mortality within 15 days despite interventions. Conclusion: VOM shows promise as a tool for assessing brainstem function and predicting outcomes in sTBI. However, its effectiveness may be limited by the use of muscle relaxants, suggesting a need for testing beyond the initial 3 days post-injury.
