Historical stigma surrounding psychosurgery has hindered innovation, but advancements in technology now allow for a reevaluation of surgical options. Magnetic resonance-guided laser interstitial thermal therapy (LITT) enables targeted ablation of specific neural circuits, representing a significant advancement in psychosurgical techniques. Summary: This editorial discusses the concept of 'connectomic ablation,' which focuses on disrupting pathological hubs within neural circuits rather than merely lesioning anatomical structures. The precision and safety of LITT, combined with high-field connectomic mapping, facilitate targeted interventions for TRD and PTSD. The authors advocate for a multidisciplinary approach to governance and informed consent, emphasizing the need for rigorous ethical standards in the deployment of this technology. Conclusion: The emergence of precision psychosurgery through LITT presents a transformative opportunity to address severe affective disorders. By redefining success metrics to include circuit normalization, the field can move beyond historical precedents and offer hope to patients suffering from debilitating conditions. A structured approach to clinical research and ethical governance is essential for the responsible advancement of this promising therapeutic option.
