Notably, sexual dysfunction, specifically retrograde ejaculation, has been associated with anterior lumbar interbody fusion (ALIF) procedures where rhBMP-2 is utilized. Methods: This article reviews existing literature and commentary on the complications associated with the use of rhBMP-2 in spinal surgery, particularly focusing on the relationship between rhBMP-2 and sexual dysfunction. The authors analyze previous studies and FDA reports to elucidate the potential mechanisms behind these complications. Results: The commentary highlights that retrograde ejaculation may occur due to damage to the superior hypogastric plexus during ALIF, leading to denervation of the bladder neck sphincter. Despite previous studies denying a causal relationship between ALIF and sexual dysfunction, the authors argue that the delayed onset of symptoms suggests a link that warrants further investigation. Conclusion: The authors assert the need for greater acknowledgment of sexual dysfunction as a potential complication of rhBMP-2 use in spinal surgery. They emphasize the importance of recognizing and addressing this issue in clinical practice and future research.
