This case serves as a reminder of the diagnostic and therapeutic challenges posed by CNS parasitic infections, prompting a reevaluation of existing management paradigms. Summary: The authors discuss four conceptual axes related to CNS sparganosis: the potential iatrogenic effects of corticosteroids on parasite behavior, the unexpected benefits of metronidazole in symptom management, the need for improved imaging protocols to track parasite migration, and the importance of developing a comprehensive immune-parasitic surveillance framework. They advocate for future research to explore these areas and improve treatment strategies. Conclusion: This case underscores the limitations of current approaches to CNS sparganosis and calls for a shift from reactive surgical interventions to a more integrated, immune-aware framework for managing these infections. By enhancing our understanding of the immunological and parasitic dynamics, we can better address the challenges posed by CNS sparganosis.
