These trials provide significant findings that may influence clinical practices in the field of surgical neurology. Summary: The DEFUSE 3 trial demonstrated that thrombectomy for patients with salvageable ischemic brain tissue within a 6 to 16-hour window significantly improved survival and functional independence compared to medical therapy alone. In contrast, the trial by Gathier et al. found that induced hypertension in patients with delayed cerebral ischemia after aneurysmal subarachnoid hemorrhage did not enhance cerebral perfusion and was associated with increased adverse effects. Conclusion: The findings from these RCTs have implications for the management of acute ischemic stroke and delayed cerebral ischemia. While thrombectomy is supported as a safe intervention within the specified time frame, the use of induced hypertension in aneurysmal subarachnoid hemorrhage requires further investigation to assess its safety and efficacy.
