Simas","João Paulo Farias"],"abstract":"Background: Sphenoid wing en plaque meningiomas are a specific type of meningioma characterized by diffuse, sheet-like dural infiltration and extensive bone hyperostosis. These tumors represent 2-9% of all meningiomas and are primarily located in the sphenoid wing, often presenting with progressive proptosis. Total surgical removal is challenging due to significant bone and dural involvement, leading to high recurrence rates, particularly with cavernous sinus extension. Methods: This study included 18 patients with sphenoid wing en plaque meningiomas who underwent surgical treatment from January 1998 to December 2008. Patients were selected based on specific criteria, including sheet-like dural involvement and extensive bone hyperostosis. Clinical records were reviewed for symptoms, surgical outcomes, follow-up, and recurrence rates, with imaging studies confirming tumor characteristics. Results: Among the 18 patients, 15 were female, with a mean age of 52.2 years. The most common symptom was proptosis, experienced by 16 patients. After surgery, proptosis improved in all affected patients, and the recurrence rate was 28% after a mean follow-up of 4.6 years, with no recurrences in patients who received immediate adjuvant radiation therapy. Conclusion: Surgical intervention for sphenoid wing en plaque meningiomas can lead to significant symptom improvement; however, the high recurrence rate necessitates careful monitoring and consideration of adjuvant therapies. Immediate postoperative radiation therapy appears to reduce the risk of recurrence.
