Al-Sharshahi"],"abstract":"Background: Anterior communicating artery (ACoA) aneurysms are the most common type of cerebral aneurysms, presenting significant surgical challenges due to their deep midline location and complex anatomy. The orbitofrontal (OF) artery can complicate the surgical exposure of superiorly projecting ACoA aneurysms as it lies within the surgical corridor. A thorough understanding of the OF artery's anatomy and its relationship to the ACoA is essential for effective surgical planning. Summary: The OF artery is a critical vascular structure that must be identified and preserved during the clipping of superiorly projecting ACoA aneurysms. Its anatomical orientation often differs from traditional 2D representations, which can obscure the view of the aneurysm and complicate dissection. The clinical implications of sacrificing the OF artery during surgery remain unclear, with existing literature lacking studies on the functional outcomes related to its preservation or sacrifice. Conclusion: The OF artery plays a significant role in the surgical management of superiorly projecting ACoA aneurysms, potentially obstructing access and increasing surgical risks. Further research is needed to elucidate the clinical consequences of OF artery sacrifice, which may inform intraoperative decisions regarding vessel preservation.
