As new imaging modalities like CT and MR scans have emerged, there has been a shift in reliance from traditional neurological examinations to these technological assessments. This transition raises questions about the impact of technology on surgical outcomes and the necessity of various imaging techniques. Methods: This article reviews the author's experiences over nearly 50 years in neurosurgery, highlighting the changes in practice due to technological advancements. It discusses specific cases to illustrate the application of imaging technologies and the decision-making process in surgical management. The article also contrasts the experiences of older and younger neurosurgeons regarding patient assessment and the use of technology. Results: The findings indicate that while technology can enhance surgical planning and execution, it does not always change the fundamental approach to surgery. In many cases, the surgical management remains consistent regardless of the additional information provided by advanced imaging techniques. The article emphasizes the importance of balancing technological reliance with clinical judgment and experience. Conclusion: The integration of technology in neurosurgery presents both opportunities and challenges. While advancements have improved diagnostic capabilities, they may not always translate to better surgical outcomes. The future of neurosurgery will depend on the ability of practitioners to discern which technologies are truly beneficial and to maintain the value of traditional clinical skills.
