Dade Lunsford"],"abstract":"Background: Functional neurosurgery has evolved significantly since its inception, with stereotactic radiosurgery emerging as a pivotal technique. Initially conceived by Leksell in 1951, radiosurgery aimed to create lesions in the brain without the invasiveness of traditional methods. Despite its potential, the lack of neurophysiologic guidance initially limited its application in functional disorders. Methods: The study involved a review of historical and contemporary applications of radiosurgery in functional neurosurgery, focusing on the development of techniques and imaging technologies. Key factors influencing lesion generation, such as dose, volume, and time, were analyzed based on both animal and clinical studies. Results: Radiosurgery has been successfully employed in various conditions, including trigeminal neuralgia, movement disorders, and psychiatric illnesses. Clinical outcomes indicate that radiosurgical thalamotomy can provide significant tremor relief, with studies showing improvement rates of up to 88% in patients treated for tremor. The refinement of imaging techniques has enhanced the accuracy and efficacy of functional radiosurgery. Conclusion: Radiosurgery represents a valuable tool in functional neurosurgery, particularly for patients unsuitable for traditional surgical interventions. As imaging technologies advance, the role of radiosurgery in treating movement disorders is expected to expand, offering a non-invasive alternative with promising outcomes.
