The increasing incidence of cranioplasties has led to a growing interest in alternative materials, such as bioceramic porous hydroxyapatite (HA), due to limitations associated with autologous bone. This study reviews complications related to custom-made porous HA implants in cranioplasty to evaluate their safety and identify potential improvements. Methods: Data were collected from clinical charts of 1549 patients who underwent cranioplasty with custom-made porous HA devices. The study analyzed postoperative complications reported by neurosurgeons and adhered to postmarket clinical follow-up regulations. An inferential statistical analysis was performed to compare complication rates between patients treated as first intention and those treated as second intention. Results: Among the 1549 patients, 72 postoperative adverse events were recorded, resulting in an overall complication rate of 4.7%. Infections were the most common complication, particularly in patients with bifrontal defects. The incidence of complications was similar between patients treated as first intention and those treated as second intention. Conclusion: The use of custom-made porous hydroxyapatite implants in cranioplasty demonstrates a manageable complication rate, with infections being the most prevalent issue. The findings suggest that while HA implants are generally safe, careful surgical technique is essential to minimize complications.
