This case highlights the potential for such errors, particularly in the context of chronic subdural hematoma surgery. The incident underscores the importance of accurate communication and verification of imaging results prior to surgical intervention. Methods: A 30-year-old male with a chronic subdural hematoma underwent burr hole evacuation under general anesthesia. The initial operation was performed on the incorrect side due to a mislabeling of the CT scan, which led to profuse bleeding from a cortical vein. A second burr hole was subsequently performed to evacuate the hematoma on the correct side. Results: Postoperative imaging revealed an area of hypodensity beneath the first burr hole, indicating a venous infarct that was not present on the preoperative scan. This complication was attributed to the accidental injury of a cortical vein during the initial incorrect surgery. The incident prompted a review of protocols to prevent future occurrences. Conclusion: This case serves as a cautionary tale regarding the potential for wrong-site surgery in neurosurgical practice. It emphasizes the necessity for multiple independent checks of critical information, including imaging and surgical site verification, to mitigate the risks associated with such errors.
